Eligibility

All active, regular full-time employees scheduled to work 30 hours or more per week are eligible to enroll in the IOTA Community Schools benefits program.

Eligible Dependents

When you enroll yourself in medical, dental, and/or vision coverage, you may also cover your eligible dependents, including:

  • Your legal spouse, common-law spouse* or domestic partner*
  • Your children up to age 26, including biological and adopted children, stepchildren, children of your domestic partner*, children for whom you are the legal guardian, and children whom you are required to provide coverage under a Qualified Medical Child Support Order (QMCSO)
  • Your children age 26 and over who have a physical or mental handicap that began before age 26

*To verify eligibility, the state where you live determines the verification documents required.

TN employees: The IOTA Community Schools Affidavit of Domestic Partnership and a document showing current relationship status such as a copy of a recurring household bill or joint statement of account dated within the last 60 days. The document must list your name, address, and your spouse’s name. If your spouse has separate finances, you may provide separate household bills or account statements, if they show the same address and are not older than 60 calendar days.

Dependent Documentation Required

You’re required to submit dependent verification documents for all benefit changes. If documentation is not provided, your dependent(s) will not be covered. Not sure what qualifies as documentation? Refer to the table below.

To cover your: You need to provide to IOTA Community Schools HR:
Spouse

A copy of a government-issued marriage certificate AND one of the following:

The first page of the most recent federal or state tax return confirming the dependent as your spouse

OR

A copy of a recurring household bill or joint statement of account dated within the last 60 days. The document must list your name, address, and your spouse’s name. If your spouse has separate finances, you may provide separate household bills or account statements, if they show the same address and are not older than 60 calendar days

Common Law Spouse The IOTA Community Schools Affidavit of Common Law Marriage and a document showing current relationship status such as a copy of a recurring household bill or joint statement of account dated within the last 60 days. The document must list your name, address, and your spouse’s name. If your spouse has separate finances, you may provide separate household bills or account statements, if they show the same address and are not older than 60 calendar days.
Domestic partner

 

Tennessee employees: You must submit a completed and notarized:

Dependent Child

For your own children: A copy of the child’s birth certificate or adoption certificate naming you as the parent

For the child of your spouse: A copy of the child’s birth certificate or adoption certificate naming your spouse as the parent AND one of the spouse documents noted above

For the child of your domestic partner: A copy of the child’s birth certificate or adoption certificate naming your domestic partner as the parent AND verification of domestic partnership

For a child through legal guardianship: A copy of the child’s birth certificate AND a copy of the court order with you, your spouse, or domestic partner as the guardian. If you are not the child’s guardian, documentation of your relationship to the guardian is also required and must be either a spouse or domestic partner.
